1. Eighteen years of age or older

5. Submit a copy of transcripts
6. Medical reference stating applicant is free of any communicable diseases and vision exam.
7. Students must submit a health report from their doctor stating they are free of any communicable diseases, including a tuberculosis test. Each student must have/have had a hepatitis B vaccine
